Opportunities for organ donation are relatively rare and the impact is life-saving, which is why it's important to give serious thought to organ and tissue donation and register your consent to donate.
By registering, you are essentially making a decision to help save lives after death, giving hope to patients waiting for a life saving or life enhancing transplant and their families.
Although most of us acknowledge that organ donation is a good thing, just over a quarter of us in Ontario are registered as donors. More needs to be done. Please make the decision to register as a donor. Once you've registered, inform your family and friends of your choice to be a donor and continue to spread the word by encouraging others to register their consent to become organ and tissue donors. Here's how to register:
Have your health card number ready. It takes very little time to register. You’ll have to provide some basic information including your date of birth and your health card number.

Click ‘Register As A Donor’. You’ll be taken to the ServiceOntario page to register your donation decision with the province of Ontario. Remember, you need to be at least 16 years of age.

Upon completion of STEP 2, your Organ and issue Donor Registration transaction is complete. STEP 3, outlined below, is not required for donor registration. It is an invitation to participate in an exciting visual arts project created by GOAL Embassy and friends.
Be sure to copy or write down the transaction reference number you receive upon completing your registration. If you'd like to participate in the GOAL Embassy Mosaic Art Project, please upload your "selfie"/photo and include the number in the submission form below. Note, the transaction reference number is a non-identifying "receipt" number and can not be used in any way to access your personal information. Saskatchewan
The red organ and tissue donor sticker on a Saskatchewan Health Services card confirms a person's wishes for donation with healthcare providers and loved ones. Stickers are available by calling the Saskatchewan Transplant Program at 1-306-655-5054 in Saskatoon.
Manitoba
To verify your registration status or to become an organ donor visit Manitoba’s Sign Up For Life website to register.
Register online or register in person at any ServiceOntario centre.
Quebec
To become an organ donor in Quebec, obtain a free donation sticker available at CLSCs, hospitals or drugstores and apply it to your health card. Transplant Quebec coordinates organ donation in Quebec. For more information phone them at 1-877-463-6366.
Nova Scotia

To become an organ donor in PEI you can do one of the following:

Place a red sticker on your P.E.I. health card. The sticker is mailed to you with your health card renewal form.

Have a red heart engraved on your driver’s license at the time of renewal. If you have already renewed your driver’s license, you can still go to the Ministry of Transportation offices and they will add it for free.

Express your wish to donate organs and tissues in writing when preparing your health care directive.
Yukon
